Job description

We are seeking an experienced Director of Rehabilitation Services to lead therapy operations across Skilled Nursing, Home Health, and Outpatient settings. This role oversees clinical quality, staffing, compliance, financial performance, and daily operations for Physical Therapy, Occupational Therapy, Speech-Language Pathology, and therapy support teams.

Key Responsibilities

• Lead and manage the therapy department across multiple care settings.
• Hire, onboard, supervise, and develop PTs, OTs, SLPs, PTAs, COTAs, and support staff.
• Ensure appropriate licensing, certifications, supervision, and continuing education.
• Oversee therapy referrals, evaluations, scheduling, documentation, and patient care.
• Maintain compliance with Minnesota state and federal regulations, Medicare, Medicaid, and other payer requirements.
• Monitor clinical outcomes, chart audits, quality initiatives, and QAPI programs.
• Manage department productivity, billing, revenue, expenses, and annual budgeting.
• Collaborate with nursing, administration, HR, billing, physicians, referral sources, and families.
• Support growth of outpatient therapy services and community referral relationships.
• Promote a positive, collaborative, patient-focused culture.

Qualifications

• Current, unrestricted Minnesota license as a Physical Therapist, Occupational Therapist, or Speech-Language Pathologist.
• Bachelor's degree required; advanced degree preferred.
• 3+ years of clinical therapy experience, preferably in SNF, post-acute, or home health.
• 2+ years of supervisory or leadership experience in therapy or healthcare.
• SNF and Home Health experience preferred.
• Knowledge of PDPM, PDGM, Medicare/Medicaid, reimbursement, compliance, and therapy documentation.
• Experience with EHR/therapy documentation systems.
• Strong leadership, communication, organization, problem-solving, and decision-making skills.
• Basic to intermediate computer skills.
