Additional Information:
- Pay: $42.00 - $47.00 Per Hour
- Shifts: Flexible

Responsibilities
- Perform thorough patient assessments by monitoring vital signs, conducting physical examinations, and documenting medical histories using electronic health record (EHR) systems such as Epic or Cerner.
- Administer medications and injections accurately following prescribed protocols while ensuring compliance with HIPAA regulations and sterile techniques.
- Assist with complex procedures including IV infusions, catheterization, wound suturing, and tube feeding, ensuring adherence to aseptic techniques.
- Support patients with specialized needs such as dementia, Alzheimer's disease, memory care, or those requiring hospice or palliative care.
- Collaborate with multidisciplinary teams to develop and implement individualized care plans that address acute pain management, behavioral health issues, or chronic conditions like infectious diseases or cardiology concerns.
- Provide education and coaching to patients and their families on health management strategies, medication administration, and discharge planning.
- Maintain accurate medical documentation of patient care activities, vital signs, specimen collection/processing, and other clinical procedures using EMR/EHR systems like Athenahealth or eClinicalWorks.
Qualifications
- Valid licensure as a Licensed Practical Nurse (LPN) with current state certification; additional certification in IV therapy or advanced practice is a plus.
- Demonstrated experience working in diverse healthcare settings such as hospitals (including Level I Trauma Centers), nursing homes, assisted living facilities, or outpatient clinics.
- Proficiency in using EMR/EHR systems including Epic, Cerner, or similar platforms for documentation and patient management.
- Strong knowledge of medical terminology, physiology, anatomy, ICD coding (ICD-9/10), CPT coding, and diagnostic evaluation procedures.
- Experience with specialized areas such as critical care (ICU), emergency medicine (ER), pediatrics (PICU), geriatrics, or home & community care is highly desirable.
- Ability to perform complex clinical tasks including venipuncture, phlebotomy, sterile processing techniques, and respiratory support such as ventilator management.
- Excellent communication skills coupled with the ability to work effectively within interdisciplinary teams while maintaining patient confidentiality in accordance with HIPAA standards.
