Job Title: Occupational Therapist (OT) – Home Health

Location: Rockford, IL (Swedish American)

Contract Duration: 13 Weeks

Shift: Day Shift (8:00 AM – 5:00 PM, Monday–Friday)

Schedule Type: Deferred Schedule

Pay Rate: $55–$60/hr (Quick Offer Available)

Guaranteed Hours: 40 hours/week

Overtime:

Daily OT after 24 hours
Weekly OT after 40 hours

Job Description:

We are seeking an experienced Occupational Therapist (OT) for a Home Health position in Rockford, IL. This role focuses on delivering high-quality patient care in a home-based setting, helping patients regain independence and improve daily functioning.

Responsibilities:

Provide occupational therapy services in a home health setting
Assess patient needs and develop individualized care plans
Monitor patient progress and adjust treatment plans accordingly
Educate patients and families on therapy techniques and goals
Maintain accurate documentation

Requirements:

Minimum 1 year of Occupational Therapy experience
Home Health experience required
Active Illinois OT License
BLS Certification
No local candidates (must reside outside 50-mile radius)
