Job Overview
We are seeking a dedicated Occupational Therapist (OT) to join a well-established rehabilitation team serving a large continuing care retirement community. This position offers a consistent weekday schedule, a collaborative work environment, and the opportunity to work with residents across multiple levels of care.
Community Overview
The community consists of approximately 400 beds and residents across:
- Long-Term Care (LTC)
- Assisted Living (AL)
- Memory Care/Dementia Care
- Independent Living Facilities (ILF)
The Independent Living community includes private homes, cottages, condominiums, a fitness center, swimming pool, and an active resident lifestyle program featuring off-site trips, sports tournaments, and recreational activities.
Responsibilities
- Evaluate and treat residents with a variety of physical, cognitive, and functional limitations.
- Develop individualized treatment plans to maximize independence and quality of life.
- Collaborate with interdisciplinary healthcare teams, nursing staff, and rehabilitation professionals.
- Maintain accurate clinical documentation and comply with all regulatory standards.
- Educate residents, families, and caregivers on therapeutic interventions and home exercise programs.
- Participate in care planning and discharge planning as needed.
Qualifications
- Active Occupational Therapist (OT) license in New Hampshire or eligibility to obtain.
- Graduate of an accredited Occupational Therapy program.
- Strong communication and interpersonal skills.
- Experience in skilled nursing, long-term care, assisted living, or rehabilitation settings preferred.
